Handling Paper Pledges

Thank you for pledging through the CFC! If you chose to make your donation using a traditional paper pledge form (or if you are a campaign worker collecting completed pledge forms from your co-workers), you will need to follow three simple steps:

  1. Double-check your pledge form for accuracy. See the instructions below. Pledge forms with missing or inaccurate information will experience processing delays and/or will potentially not be processed. 
     
  2. Download, print, and complete the CFC Pledge Summary Report Form (if you are submitting more than one form). 
  3. Mail the pledge forms and the CFC Pledge Summary Report form to (remember, the CFC does not accept cash donations):

Get Pledge Forms

The time to donate through paper pledges has ended, however if there are still outstanding from your agency, please be sure to send them in as soon as possible. 

Paper pledges will continue to be processed through mid-February and will be added to agencies' final CFC tallies - so be sure to mail your paper pledges in today!

A. CFC unit code and the ZIP Code for your unit/office are correct

B. Overseas Employees box should only be checked if you are physically located in an overseas military command (AFRICOM, CENTCOM, EUCOM, INDOPACOM, SOUTHCOM).

C. Reporting unit information (Department, Agency, Office) is complete and correct

D. Social Security number is required for a payroll deduction pledge (can be left blank for a one-time check pledge)

E. Total gift and charity designation amounts should match

F. Funds must be designated using a 5-digit charity code (undesignated pledges are not accepted)

G. Authorization portion must be signed

H. The information release section is optional - leaving it blank is an anonymous gift to the charities
